Before anyone is seen or treated we will need to speak with you discuss some of your history and knowledge as well as get your basic information to send over all consent forms and intake paperwork before any treatment or appointments will be made.

These consents for treatment are vital in the process of treatment and keeping all HIPAA standards up to date.

After three missed appointments with no call or heads up to cancel, your place as a patient will be terminated and you will be discharged from our practice, as well as if we find out you are doctor shopping, taking street medication on top of what we prescribe or selling your medication showing no changes in your lab work.

If the required labs are not done in-house or at a lab or other office and sent to us in a timely matter, treatment will not be started or continued without knowing where your body stands, not complying with this could lead to major risks and be putting yourself into danger, we need these results to monitor your body, how it handles the medication and keep your body functioning at its absolute best.

Non-Compliance will not be tolerated and you will be expected to follow the physicians instructions exactly as given, we do what we do for a reason, we know what is needed and why as well as how it will help you, trust the professionals.
